JOB SUMMARY:

We are seeking a dynamic, passionate Registered Nurse (RN), who wants to be part of an interdisciplinary virtual team in home and community care by providing clinical leadership and support to direct care providers, including nursing, personal support, and rehabilitation teams, across the province, as well as virtual care delivery to clients in a truly client-centered approach.

 

The Clinical Practice Resource Team (CPRT) provides 24/7, 365 days a year virtual clinical support across all SE Health business lines, supporting safe, high-quality care delivery regardless of discipline, geography, or time of day.

 

You will have opportunities to work closely with the Professional Practice leadership team in supporting excellence in practice through knowledge translation. You will use evidence-informed clinical knowledge, skill, and judgment while supporting optimal scope of practice across disciplines to deliver holistic care delivery that meets the physical, safety, emotional, and spiritual needs of clients and their families in their homes and communities.

 

This is Full Time 12 months contract opportunity until August 2027, offering the chance to gain new experiences and contribute within SE Health's Virtual Clinical Support Team

 

CPRT practice is grounded in four core domains:

  • Just-in-time clinical decision making

  • Telepractice

  • Knowledge translation

  • Complex care planning 

 

These domains apply across all disciplines and business lines within SE Health.

As part of the Clinical Practice Resource Team, you will:

  • Just-in-Time Clinical Decision-Making:

    • Use your extensive clinical knowledge of home and community care, SE Health policies and procedures, and best practices to direct care providers (RNs, RPNs, personal support, and rehabilitation staff) who require timely clinical decision-making support related client care.

    • Respond to real-time clinical questions, including changes in client condition, medication support, IPAC, and application of policies and procedures.

  • Telepractice and Virtual Care Delivery:

    • Deliver assessment, management, and virtual care to clients, as well as educational support to caregivers through digital technology.

    • Support the integration of virtual care across nursing, personal support, and rehabilitation services.

  • Knowledge Translation and Practice Support:

    • Partner with Advanced Practice Leaders to develop evidence-based clinical practices for home and community clients.

    • Liaise with regional Practice Coaches and Clinical Managers to support the translation of clinical best practices to the front-line.

    • Utilize your knowledge to support the development of care plans and changes to treatment codes, as required.

    • Assist the Advanced Practice leadership team with reviews and development of policies and procedures.

  • Complex Care Planning:

    • Support complex care planning through collaboration with interdisciplinary teams.

    • Monitor your own practice and the needs of our clinical staff through analysis of reports.

    • Contribute to innovation in supporting the organization to achieve its goals.

    • At times, assist with follow-up with funders and/or physicians to ensure that referrals and service updates are complete.

    • Demonstrate commitment to continuous learning and professional development through participation in formal and informal learning opportunities.

    • Demonstrate a commitment to interdisciplinary care and a professional practice model through partnerships with the Advanced Practice leadership team.

 

REQUIREMENTS:

  • Current Certificate of Competence with the College of Nurses of Ontario

  • Master of Nursing, preferred, Bachelor of Science of Nursing required.

  • Current expertise/certifications in Older Adult/Geriatrics, Chronic Disease Management, General medicine/surgical, Infusion Therapy, preferred.

  • Candidates with recent pediatric experience preferred.

  • Candidates with Canadian Nurses Association or North American certifications, preferred.

  • A minimum of 3-5 years of recent community nursing experience with a comprehensive knowledge of the organization’s programs and services

  • Demonstrated ability to support interdisciplinary teams, including nursing, personal support, and/or rehabilitation services

  • Comfortable using technology, including telephone systems, software programs, and computer programs.

  • Private room with a closed door in which you will work including high speed internet connection.

  • Demonstrate ability to be flexible and adaptable with excellent organizational skills.

  • Demonstrate ability to take initiative, show leadership and adapt to rapidly changing demands.

  • Excellent interpersonal communication and utilization of problem solving and critical thinking skills.

  • Sound decision making skills to plan, organize and implement proactively and responsively.

  • Demonstrated ability to work independently and to act as a collaborative team member.

  • A demonstrated appreciation for Quality Improvement processes.

  • A willingness to attain required SE Health certifications.

  • Availability to work: days, evenings, nights and weekends.

  • Willingness to be cross trained to other services offered within the Center of Excellence in Virtual Care.

  • Vehicle and valid driver’s license, with the ability to travel and attend in-person meetings, when required.

  • Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.

The total compensation for this position is $69,000 - $87,000.
