Position summary: - 6 month contract

Reporting to the Regional Director, the successful candidate provides leadership in the implementation of a robust quality, client safety and risk management program within the Service Delivery Centre. The Director of Nursing and Personal Support fosters a culture of continuous quality improvement and excellence by working with partners to develop and enhance systems and processes that embed dimensions of quality across the portfolio. A strong focus on leading practices, standards, client-centeredness, contract management and safety are paramount to support the organization’s strategic priorities and align with the overall vision and mission of the organization.

Responsibilities:

  • Supports the Regional Director in strategic dialogue with internal and external partners and stakeholders to drive ideas and strategies for advancing collective quality practices and outcomes.
  • Lead for continuous quality improvement of safe quality home and community client care and client/caregiver experience, adopting the quintuple aim of quality framework and leading practices for measurable results and enhancing performance driven culture
  • Fulfills the regulatory college’s mandate for client care as the highest level of clinical leadership at the local site
  • Leads Accreditation preparation at the site level and contributes to organization’s activities to ensure an Exemplary status of achievement.
  • Works in partnership with the Regional Director, the Professional Practice Leader and the site leadership teams to drive a culture of quality improvement and client safety.
  • Supports the preparation and presentation of monthly and quarterly quality and safety material for review with the Regional Director, and with external funders and partners.
  • Leads partnership management with HCCSS funders, regional OHT, and other future potential partners.
  • Contributes to the development and execution of the SE Health’s HOPE Clinical Model of Care at the site, ensuring sustainability of practices for long-term success.
  • Contributes to business development activities.
  • Models leadership attitudes and behaviors consistent with SE Health values, including a relentless focus on person-centered care, excellence, accountability, collaboration, equity, and innovation
  • Provides performance management of the nursing, and personal support leadership team, including the ongoing evaluation of performance and the development of skills and competencies required to achieve SE Health’s vision and strategies
  • Participate with funders as required to ensure contractual obligations are being met
  • Drive processes and metrics forward in alignment with the business strategy (example: lost revenue, missed care, client and employee satisfaction etc.)
  • Facilitate and maintain team development and succession planning.
  • Supports local leadership team in the coaching and managing of staff with performance issues and consult appropriately with HR

Requirements:

  • Master’s degree in Nursing or Health Care Management is required (consideration will be given for candidates in the last half of the program completion)
  • A regulated health care professional designation (Registered Nurse) required, holding a current professional license, in good standing, to practice in Ontario
  • A minimum of 5-7 years of leadership/management experience in a health care organization.
  • Recent and relevant clinical knowledge and expertise of Older Adult care needs
  • Ability to promote and facilitate evidence-based practice within a community setting
  • Demonstrated effective critical thinking, analytical, decision making and problem-resolution skills
  • Demonstrated expertise and ability to apply, teach, coach and mentor individuals and teams
  • Maintains accountability within the team to achieve work priorities within stated timelines
  • Superior communication skills and ability to communicate effectively to direct and motivate team and others, maintaining a persuasive and credible presentation style at all levels of the organization
  • Promotes and consistently demonstrates an open and collaborative leadership style that empowers high team functioning
  • Ability to influence others and able to establish credibility with all levels of the organization, challenging the status quo and being a catalyst for positive change
  • Ability to distill complex information into clear, concise messages and execute ideas into impactful actions
  • Successfully lead and engages team to achieve results
  • Excellent organization, planning and time management skills
  • Ability to manage multiple, often competing priorities
  • Excellent interpersonal and verbal/written communication skills.
  • Knowledge of quality and risk management practices.
  • Complete MS Office and Procura familiarity is required.
  • Ability to travel within the province of Ontario, with occasional requirements for overnight stays.
  • A vehicle, valid driver’s license with travel required.  Must be willing to travel in person in York region.. Frequent attendance in person at Group Practices will be a requirement. Must be willing to come to the Markham office twice a week.
