JOB SUMMARY:  

We are seeking a dedicated healthcare professional to ensure high-quality client care through the supervision of Community Health Workers (CHWs) and Licensed Practical Nurses (LPNs). This hybrid role includes routine community visits and close collaboration with interdisciplinary teams. All responsibilities must align with BCCNM standards and the legislative scope of practice for home and community care. Occasional travel between Vancouver/Victoria regions and to the island.

Key Responsibilities:

A. Quality Service Delivery

  • Support the Care Manager overseeing frontline staff and client care services.
  • Conduct client assessments, supervisory visits, and update care plans as needed.
  • Communicate with clients, caregivers, and families to address care needs and feedback.
  • Investigate and address care concerns and staff practice issues in a timely manner.
  • Provide information on community resources.
  • Participate in the on-call rotation.

B. People and Team Development

  • Support orientation, training, and performance management of frontline staff.
  • Collaborate with care coordinators and leadership, offering backup support as needed.
  • Participate in coaching conversations and practice reviews with staff.

C. Documentation & Reporting

  • Maintain accurate electronic records of care and communications.
  • Stay current on organizational policies and recommend updates when appropriate.
  • Assist with accident investigations (e.g., WorkSafe BC) and complete the required documentation.
  • Enter incidents, compliments, and complaints into the RL6 reporting system.

D. Additional Responsibilities

  • Perform other related duties as assigned by the leadership.

 

Qualifications:

  • Current LPN registration in good standing with BCCNM (full scope required).
  • Supervisory experience in healthcare settings.
  • Current CPR certification and full immunization record, including COVID-19.
  • Clear and recent (within 12 months) vulnerable sector check.
  • Excellent verbal and written communication skills.
  • Proficiency with digital tools (computers, mobile apps).
  • Strong organizational, problem-solving, and leadership skills.
  • Self-motivated with the ability to work independently and as part of a team.
  • Physically and mentally capable of fulfilling job duties.
  • Valid Class 5 driver’s license and reliable vehicle required.
