We are seeking a dynamic and passionate Personal Support Worker (PSW) who wants to be part of the transformational change in home and community care by providing care in a truly client-centered approach. You are excited about working in an environment that puts the care team in the driving seat; being part of a self-directed team that offers a high level of professional autonomy and responsibility. You will be part of a diverse care team that will provide services to a roster of clients in the community. You be part of a collaborative team that will conduct joint care planning with a focus on client-centered goals addressing client’s physical, cognitive, social, emotional, and spiritual domains, creating a holistic care plan. Each client is assigned a primary care clinician, a member of the interdisciplinary team who will lead care planning for that client and facilitate interdisciplinary team to reach client’s care goals. As part of the SE Transitions team, you will be a part of the team that will be accountable for the care and services you provide and the outcomes your clients achieve.

Qualifications accepted to work as a PSW:

  • Valid Certificate as a Personal Support Worker (PSW)
  • RN or RPN Nursing student (2 clinical placements completed)
  • Valid Developmental Support Worker
  • Foreign trained healthcare professional - Nurse or Physician

Requirements:

  • current standard First Aid / CPR level C Certificate 
  • A valid driver’s license and vehicle.
  • Current immunization record (TB test completed within the past 12 months) is required upon hire
  • Current vulnerable sector check screening is required upon hire
  • Availability to work days OR evenings; including weekends
  • Able to work independently as well as part of a team, with excellent organizational skills to manage your schedule
  • Well-developed interpersonal and customer service skills, to effectively communicate with clients and to provide companionship to those who need it
  • Effective problem-solving and conflict management skills
  • Basic computer/smartphone skills would be an asset.
  • This is a physically demanding position and requires hand, arm and shoulder dexterity, sensory ability, lifting to a maximum of approximately fifty-five (55) pounds, walking, climbing stairs, standing, bending, reaching, and frequent movement

To be eligible for the PSW ROS, a Personal Support Worker must:

  •  Successfully completed an Ontario PSW training program on or after April 1, 2023, at a public-assisted college, private career college, district school board or Indigenous Institute that meets the 2022 program standard established by the Ontario Ministry of Colleges and Universities Ontario Ministry of Training, Colleges and Universities program standard
  • Begin employment with a participating employer between April 1, 2023, and March 31, 2026; and
  • Commit to 12 months of employment at a full-time work schedule
