Position Summary:

Physiotherapist: Mackenzie Health Outpatient Stroke Program

We are currently seeking a Physiotherapist to work in Stroke Outpatient Clinic located in Vaughan (955 Major Mac). Physiotherapist will provide patient management through all stages of assessment, treatment, consultation and follow-up services to patients.  The successful candidate will be familiar with working in an outpatient clinic setting.

As a successful candidate, you are a graduate of an accredited program in Physiotherapy and are a member in good standing of the College of Physiotherapists of Ontario.  You demonstrate excellent interpersonal, organizational and communication skills, both written and verbal, along with a high level of initiative and adaptability to rapidly changing demands.

Responsibilities:

  • As a Physiotherapist (PT) you will be conducting assessments & treatment plans
  • As a PT you will be formulating goals and recommendations
  • Planning and implementing patient centered, goal-directed treatment programs & working with Therapy Assistants
  • Assist in the promotion of physiotherapy services to external referral sources
  • Participating actively in the planning, implementation and evaluation of the program

 Requirements:

  • Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively with a high-performance interdisciplinary rehabilitation team
  • Graduate of an accredited program in Physiotherapy and are a member in good standing of the College of Physiotherapists Ontario
  • 2 years of clinical experience in Stroke/Neurological patient populations is preferred
  • Experience in assessment and generating treatment plans for patients with stroke/neurological disorders
  • Knowledge of stroke best practices
  • Experience with Chronic disease management is a good asset
  • Sound knowledge of community resources is an asset
  • NDT training is an asset
  • Effective interpersonal, organizational and communication skills
  • Strong customer service focus and excellent problem-solving skills required
  • A positive work and attendance record is essential
  • Ability to adapt to new computer applications
  • CPR Level C + AED, and Standard/ Intermediate First Aide certificate
  • Valid G license required
  • Criminal background check, Vulnerable sector check and Immunization record (current)
