About Journey Home Hospice Windsor

We’re proud to share that Journey Home Hospice Windsor is growing, evolving from a successful pilot launched in 2022 into a permanent program providing high-quality, compassionate, and trauma-informed end-of-life care to people who are structurally vulnerable in the Windsor-Essex community.

Launched in partnership with Erie St. Clair Service Delivery Centre and as a satellite site of Journey Home Hospice Toronto, Windsor’s four-bed hospice, including three residential beds and one respite bed, has become a proof of concept for how health care, community partners, and compassion can come together to serve people experiencing homelessness and other forms of marginalization.

Now operating as a permanent program of the Saint Elizabeth Foundation, in collaboration with Assisted Living Southwestern Ontario (ALSO) and the City of Windsor, we are looking to expand our nursing team and welcome professionals who want to make a meaningful impact in the lives of those often excluded from traditional hospice care.

Job Description

Many individuals experiencing homelessness or structural vulnerability do not have access to family, housing, or community supports at end-of-life. Journey Home Hospice Windsor is a 24/7 residential hospice program that provides a welcoming and safe place for these individuals to receive comfort, dignity, and holistic care, until natural death.

As an RPN at Journey Home Hospice Windsor, you will be part of a small but highly dedicated interprofessional team that includes Registered Nurses, PSWs, physicians, social workers, spiritual care providers, and volunteers. Together, you will deliver trauma-informed, harm-reducing, and culturally safe palliative care that meets each patient where they are.

Requirements

·        Current Certificate of Competence with the College of Nurses of Ontario as a Registered Practical Nurse (RPN)

·        LEAP Certificate (or willingness to complete within 6 months of hire)

·        At least 1 year of hospice palliative care experience preferred

·        Experience working with structurally vulnerable populations (e.g., homelessness, mental health, substance use, poverty)

·        Mental Health Nursing Certification or equivalent training is an asset

·        Familiarity with Electronic Medical Record (EMR) systems; experience with PointClickCare is strongly preferred

·        Willingness to contribute beyond clinical duties, including:

o   Coordinating with hospitals, OHT/OHaH, and Toronto-based support teams

o   Supporting administrative tasks and documentation

o   Assisting with supply and equipment ordering and management

o   Participating in interdisciplinary communications and quality improvement

·        Excellent verbal and written communication skills, with an ability to build respectful relationships with patients, families, volunteers, colleagues, partners, funders, and donors.

·        Strong interpersonal and organizational skills

·        Ability to work both independently and as part of a collaborative care team

·        First Aid and CPR certification

·        Availability to work 12-hour shifts (days/nights/weekends)
