JOB SUMMARY:
The Senior Practice Consultant, Hospice & Palliative Care provides senior-level clinical expertise and leadership across all clinical programs of the Saint Elizabeth Foundation. As a senior registered nurse with advanced knowledge in hospice, palliative, and community-based care, this role supports high-quality, evidence-informed care delivery, professional development, and clinical innovation. The position collaborates with operational teams, program leads, and external stakeholders to enhance care models, set clinical standards, and guide organizational strategy in alignment with the Foundation’s mission.
Full Time 12-month contract
JOB RESPONSIBILITIES:
Clinical Leadership & Program Oversight
Provide senior clinical guidance across all Foundation programs, including hospice, palliative, and community-based care.
Support program design, implementation, and evaluation of clinical services to ensure quality, accessibility, and sustainability.
Serve as a consultant for complex or high-acuity cases, supporting staff and leadership in care planning and decision-making.
Practice Standards & Evidence Integration
Establish and maintain clinical practice standards, policies, and workflows across Foundation programs.
Integrate evidence, best practices, and quality metrics into care delivery and program evaluation.
Collaborate with analytics and business intelligence teams to inform program planning and outcomes measurement.
Education & Workforce Development
Lead clinical mentorship, supervision, and professional development initiatives for staff and learners.
Design and deliver educational programs to support workforce competency in hospice, palliative, and end-of-life care.
Promote adoption of digital and virtual care practices to enhance reach and equitable access to services.
Strategic Consultation & Innovation
Provide thought leadership to advance the Foundation’s clinical priorities, including equitable access, innovation, and integration of spiritual care and grief support.
Collaborate with internal and external partners to strengthen the Foundation’s position as a leader in hospice and palliative care.
Quality, Risk, and Accreditation Support
Contribute to the development and monitoring of quality indicators, risk frameworks, and accreditation readiness for all clinical programs.
Advise on continuous improvement strategies and compliance with organizational and regulatory standards.
QUALIFICATIONS:
Master’s degree in Nursing (MN) or related clinical discipline; undergraduate nursing degree required.
Current RN licensure in good standing with provincial regulatory body.
7+ years of clinical nursing experience, including hospice, palliative, or community-based care.
Demonstrated leadership experience in program design, clinical consultation, or enterprise-level service integration.
Preferred: Doctorate degree, Certified Healthcare Executive, or recognized certifications in hospice/palliative care, leadership, quality, or project management.
