JOB SUMMARY:
We are seeking a dedicated healthcare professional to support the delivery of high-quality client care within our Home and Community Care programs across Ontario. In this hybrid role, the Team Lead will conduct routine community visits, participate in care coordination activities, and collaborate closely with interdisciplinary teams to ensure services align with the standards and legislative requirements set by CNO.

Key Responsibilities:
A. Quality Service Delivery
- Support the Care Manager overseeing frontline staff and client care services.
- Conduct client assessments, supervisory visits, and update care plans as needed.
- Communicate with clients, caregivers, and families to address care needs and feedback.
- Investigate and address care concerns and staff practice issues in a timely manner.
- Provide information on community resources.

- Perform other related duties as assigned by the leadership.
Qualifications:
- Current member of the College Nurses of Ontario (RPN/RN accreditation)
- Supervisory experience in healthcare settings.
- Current CPR certification and full immunization record, including COVID-19.
- Clear and recent (within 12 months) vulnerable sector check.
- Excellent verbal and written communication skills.
- Proficiency with digital tools (computers, mobile apps).
- Strong organizational, problem-solving, and leadership skills.
- Self-motivated with the ability to work independently and as part of a team.
- Physically and mentally capable of fulfilling job duties.
- Valid Class G driver’s license and reliable vehicle required.
