POSITION SUMMARY:

We are looking for Lead PSW’s to work with the Personal Support Manager to assist them with initial admission visits.

  RESPONSIBILITIES:

·         Provide care using the OCAR framework

·         Conduct Tug test on admission

·         Complete the Braden scale on admission

·         Complete all documentation as required

·         Initiate first contact with client and or family

·         Develop care plan based on observation and screening tools, interview with clients and or family

·         Document client’s diagnosis, functional capabilities and base line as per documentation

·         Contact and communicate with PSM as required to initiate care, request for additional services and provide safe care

·         Acts as a resource in care of the older adult for other PSW’s

 

REQUIREMENTS:

·         PSW certification

·         Minimum 1 year experience as a Preceptor or leadership/management is preferred

·         Minimum 1 year experience as a PSW at Saint Elizabeth is required

·         Takes initiative and is an advocate for the client/family

·         Promotes a philosophy of Person and Family Centered Care

·         Promotes independence in patients/families and involves them in their plan of care

·         Excellent interpersonal and verbal/written communication skills

·         Excellent time management skills

·         Thrives in a fast-paced environment, with multiple competing priorities.

·         Outcome-oriented and able to achieve desired results. 

·         An ability to deal with conflict and excellent problem-solving skills

·         Experienced in home care visits

·         Able to adjust to changes in schedule and expectations

·         Able to recognize and report risk situations

·         Excellent judgment and observation skills

·         Experience/education with the older adult population

·         Interested in learning

·         Interested in leaderships opportunities

·         Accurate, timely and complete documentation
