JOB SUMMARY:  

The Spiritual Care Provider delivers compassionate, evidence-informed spiritual health, grief, and bereavement care to patients, families, caregivers, and staff across SE Health service lines. Working within an interprofessional team, the provider conducts spiritual assessments, provides counseling and support during illness, loss, and transition, and contributes to holistic care planning.

The role maintains an active clinical caseload, delivering care through virtual and in-person encounters as required. In addition to direct patient and family care, the provider contributes to team consultation, supports staff well-being following difficult clinical situations, and assists with the mentorship of Clinical Pastoral Education (CPE) students and interns.

Full time 12 Month Contract 

JOB RESPONSIBILITIES:

Direct Patient, Family & Caregiver Care

  • Provide spiritual health and bereavement care to patients, families, and caregivers experiencing illness, grief, loss, or life transitions.

  • Conduct spiritual assessments that consider emotional, cultural, existential, and relational dimensions of care.

  • Provide spiritual counseling, supportive listening, and meaning-making support during times of crisis, illness progression, and end-of-life care.

  • Provide anticipatory grief support and bereavement follow-up with families after death.

  • Facilitate or support grief groups, memorial services, or rituals when appropriate.

  • Maintain a consistent clinical caseload across SE Health service lines.

 

Interprofessional Collaboration & Consultation

  • Participate in interprofessional care planning and team discussions to support holistic care delivery.

  • Provide consultation to clinicians and staff regarding spiritual, cultural, and grief-related concerns.

  • Support staff following patient deaths, critical events, or emotionally challenging situations.

  • Build collaborative relationships with clinical teams across programs and service lines.

 

Documentation, Virtual Care & Practice Standards

  • Complete timely and accurate documentation within SE Health electronic systems.

  • Use standardized spiritual assessment tools and documentation practices.

  • Deliver care through virtual platforms and telehealth tools as appropriate.

  • Ensure privacy, consent, and professional documentation standards are maintained.

 

Education, Mentorship & Quality Improvement

  • Support the learning environment for Clinical Pastoral Education (CPE) students and interns through mentorship and modeling of practice.

  • Participate in team meetings, case consultations, and reflective practice.

  • Contribute to quality improvement activities and service development initiatives.

  • Maintain professional development and participate in supervision or reflective practice.

 

QUALIFICATIONS:

Required

  • Master’s degree in Theology, Divinity, Pastoral Studies, Spiritual Care, or a related field.

  • Completion of Clinical Pastoral Education (CPE) units or equivalent training.

  • Experience providing spiritual care and grief support in healthcare or community settings.

  • Demonstrated ability to support patients and families experiencing illness, loss, and grief.

  • Strong communication, assessment, and interpersonal skills.

  • Ability to work independently within an interprofessional team environment.

  • Experience using telehealth platforms and electronic documentation systems.

 

Preferred

  • Professional certification or membership with organizations such as CASC or NACC.

  • Experience in hospice, palliative care, home care, or community-based care settings.

  • Experience facilitating grief or bereavement groups.

  • Experience mentoring students or interns.

  • Bilingual (English/French) considered an asset.
