Job Summary

The Change Manager supports the delivery of change across priority transformation initiatives at SE Health. This role works closely with program teams, leaders, and business stakeholders to translate project activity into clear impacts, actions, and supports for affected teams. It is responsible for helping people prepare for, implement, and stabilize change through a combination of planning, stakeholder engagement, readiness support, communications and learning coordination, and feedback-driven adjustment.

The role is suited to someone who is organized, thoughtful, and able to work effectively in a fast-moving environment where plans evolve and close coordination with delivery teams is essential. Success in the role requires strong execution discipline, sound judgment, and the ability to translate evolving program direction into practical steps for stakeholders.

Key Responsibilities

Change Planning and Delivery

  • Develop and manage change plans aligned to assigned initiatives, workstreams, or implementation priorities
  • Translate evolving program direction into clear, actionable plans for stakeholders and impacted teams
  • Work with program leads, subject matter experts, and business stakeholders to understand impacts, dependencies, and implementation requirements
  • Support change activity across planning, implementation, and early stabilization phases
  • Keep change plans aligned to actual delivery timelines, decisions, and operational realities

 

Readiness, Impact, and Stakeholder Support

  • Conduct stakeholder, impact, and readiness assessments for affected teams, roles, and functions
  • Help identify where additional engagement, support, clarification, or reinforcement may be required
  • Track readiness indicators, implementation issues, and stakeholder feedback
  • Surface issues that may affect implementation early, including gaps in alignment, clarity, or readiness
  • Support mitigation planning and follow-through with delivery and business teams

 

Communications, Learning, and Engagement Coordination

  • Support the design and coordination of communications, engagement, and learning activities based on the needs of impacted audiences
  • Work with leaders and partners to tailor messages and supports for different stakeholder groups
  • Help make change activity understandable, relevant, and well sequenced for those impacted
  • Gather, synthesize, and apply feedback to improve implementation approach over time

 

Delivery Coordination and Continuous Improvement

  • Work closely with delivery teams and business stakeholders to keep change activity connected to actual implementation needs
  • Maintain discipline across milestones, deliverables, actions, and follow-through
  • Use common methods, tools, and templates in a practical and consistent way
  • Share lessons learned and identify opportunities to improve how change is supported across initiatives
  • Contribute to a collaborative, accountable, and solutions-oriented team environment

 

Qualifications & Experience

Required

  • 5+ years of experience supporting change, transformation, implementation, or operational improvement initiatives
  • Experience supporting change within digital, process, operational, or implementation-heavy initiatives, and working with multiple stakeholder groups to support readiness and adoption
  • Strong planning, coordination, facilitation, and communication skills
  • Ability to work in a structured way while adapting to shifting priorities and evolving direction
  • Strong interpersonal skills and ability to build credibility with leaders, teams, and delivery partners
  • Experience managing multiple deliverables or workstreams in a dynamic environment

 

Preferred

  • Experience in healthcare, social services, or other complex service environments
  • Experience supporting digital, process, or operational transformation initiatives
  • Experience in change management, organizational development, communications, learning, or implementation disciplines
  • Formal training, education, or certification in change management or a related field

The total target compensation for this position is $87,000 - $108,000.
